A cooperative program between the University of Vermont Extension and
the VT Division of Vocational Rehabilitation. We provide services to individuals

Medicaid's health program for children and young adults up to age 21.
Its goal is to promote healthy behaviors and assure access to quality
medical and dental care. VDH staff contact eligible particpants and provide
information about what health services are available through Medicaid
or Dr. Dynasaur and why these services are important. Services are coordinated
with and through WIC, school supervisory unions, SRS, and other groups
that work with children and adolescents. Services include contacting the
families or gurdians of children enrolled in Med.?Dr. D. through mailings,
telephone, or home visits, collaborate with medical and denal providers
to ensure that children receive al necessary and available services, educate
familes about proper car seat usage, educate parents about baby bottle

Babies Kids and Families (HBK&F) at the Vermont Department of Health,
Newport District Office
The goal is to improve the health of pregnant women and infants receiving
Medicaid services by assuring the availability and accessibillity of health
and social services. Healthy Babies nurses work with doctors and other
health care providers to assure pregnant women and babies receive health
assessments and that all social and health service providers are working
together to coordinate care. They help determine the needs of women and
infants in the program and help them access needed services. Home visiting
services are provided to families by the staff of the Health Department,
Home Health agencies, and Parent Child Centers. In these visits nurses,
nutritionists, or family support workers answer questions about pregnancy,
delivery, parenting; talk toparents about infant health needs like immunizations
as well as health risks, and help families learn about and access community
services and ongoing medical care.

R educe the incidence of infectious diseases through immunization, monitoring
disease reports, community education and outbreak investigations. Activities
include: distributing free chlldhood and adolescent vaccines and some
free adult vaccines to health care providers, educating parents about
the importance of childhood immunizations and keeping an upt-to-date recore,
review the immunization records of children and provide information and
referral for those needing immunizations to health care providers, provide
information and vaccines to people who need them through work with community
organizations, work with health care providers on appropriate dosage,
use and storage of vaccines, help coordinate free immunization clinics
with schools, hold special immunization clinics in disease outbreak situations,
investigate reported cases of infectious diseases such as measles, hepatitis
and rabies among others to determine the cause and recommend measures

Poisoning Prevention Program at the Vermont Department of Health, Newport
District Office
Goal is to reduce the number of lead poisoned chlidren, increase screening
and help identify and eliminate potential sources of lead. Activities
include: tracking the number of children screened and the number of positive
diagnoses, promote professional and public education about the risks of
lead poisoning, the importance of screening and screening methods, for
severely lead poisoned children arrange environmental ssessmensts of homes
and day cares, and monitor follow-up to make sure children receive apropriate
services, train health care providers to perform capillary blood lead
screening tests, offer free capillary screening for one and two year olds
on WIC or by special request if appropriate.

Infants Children Program (WIC) at the Vermont Department of Health, Newport
District Office
The goal is to improve health by teaching families about good nutritional
practices and providing nutritious foods to eligible Vermonters. WIC serves
children less than 5 years of age, pregnant women, mothers of infants
less than 6 month old, and breastfeeding women of infants less than one
year old. Eligibility involves income less than 185% of poverty and at
least one medical or nutritional condition that would be improved by health
and nutrition education or the the supplelmental foods provides. Women
or children with Medicaid or Dr. Dynasaur are automatically income eligible
for the Program. WIC clinics are held throughout the district and include
health education and nutrition counseling, screening for selected health
problems, development of a supplemental food package that can include
cereals, formula, milk, juice, cheese, eggs, peanut butter, beans, and
carrots. Food packages are delivered to families" homes by private
WIC vendors. Women who are breastfeeding receive additional support and
educational materials.

MedQuest is a program of the Northeastern Vermont Area Health Education
Center. The program serves up to 40 Vermont 9th and 10th graders each
summer from our service region of Caledonia, Essex, Lamoille, Orange,
Orleans and Washington counties. MedQuest is a one-week residential health
career exploration program that takes place at Johnson State College and
Lyndon State College. Participants have the opportunity to job shadow
health care professionals, receive CPR certification, first aid training
and attend a variety of presentations related to health careers and personal
and professional development. First year medical students from UVM staff

MBDP offers free services to income eligible Vermont who are small business
owners or prospective business owners. Our service area includes Orleans,

The Outreach Department assists low income residents get the help they

- Work & Self Reliance Department
The NEKCA's Work & Self Reliance Department works with people with
little or no work history, or with barriers to employment. The Department
offers opportunities for these people to overcome these issues and move
into the work force. W & SR houses the Careers Program, the Work Placement
and Training program, and the Kingdom Cars Program. Careers is a workshop
program that runs 7 weeks long, 5 times a year. It offers educational
and hands on work training. The Work Placement and Training Program runs
all year, and offers hands- on work training. Kingdom Cars is a donated
vehicle program. Cars are fixed and sold for the cost of the repairs.
This program is used to help people get to work.

- Work & Self Reliance Department
The NEKCA's Work & Self Reliance Department works with people with
little or no work history, or with barriers to employment. The Department
offers opportunities for these people to overcome these issues and move
into the work force. W & SR houses the Careers Program, the Work Placement
and Training program, and the Kingdom Cars Program. Careers is a workshop
program that runs 7 weeks long, 5 times a year. It offers educational
and hands on work training. The Work Placement and Training Program runs
all year, and offers hands- on work training. Kingdom Cars is a donated
vehicle program. Cars are fixed and sold for the cost of the repairs.
This program is used to help people get to work.

NEKCA CFDP Head Start/Early Head Start offers comprehensive early childhood
and family development services for parents with children prenatal through
